Overview
A Kiosk allows a client to complete clinical documents independently. In-office computers/tablets, the Client Portal and a client's personal email can each serve as a kiosk for this purpose. This article describes how clinical documents may be sent to each of these three options.
Clinical documents can be sent from:
Send Clinical Documents from the Appointments Module
- Click on the Appointments module.
- Locate the client's appointment.
- Click on the appointment to highlight.
- The details of the appointment and client information will appear in the appointment side-panel to the right.
- Click Have client fill out form at home/on kiosk link.
- If you do not see this option, you may still need to configure your info panel.

- The Assign Client to Fill out Form window will open.
- Select the radio button adjacent to the available Kiosk option being used.
- Verify or select the appropriate Program.
- Choose a Form Group or one or more Form(s) to send to the kiosk.
- Click OK.
- If a computer or tablet kiosk was selected, that computer/tablet will display the clinical document.
- If more than one clinical document was sent, the left margin on the kiosk will display a series of clinical document numbers. The checkbox will display as selected once that clinical document has been completed and submitted.
- If more than one clinical document was sent, the left margin on the kiosk will display a series of clinical document numbers. The checkbox will display as selected once that clinical document has been completed and submitted.
NOTE: Once the clinical document has been completed by your client, you will find this document in the Clinical/Charting Module.
Send Clinical Documents from the Clients Module
- Click on the Clients module.
- Locate the client from the available client list.
- Click the kiosk link adjacent to the client record on the far right.
- The Assign Client to Fill out Form window will open.
- Select the radio button adjacent to the available Kiosk option being used.
- Verify or select the appropriate Program.
- Choose a Form Group or one or more Form(s) to send to the kiosk.
- Click OK.
- If a computer or tablet kiosk was selected, that computer/tablet will display the clinical document.
- If more than one clinical document was sent, the left margin on the kiosk will display a series of clinical document numbers. The checkbox will display as selected once that clinical document has been completed and submitted.

Send Clinical Documents from Clinical/Charting Module
- Click on the Clinical/Charting module.
- Using the available filters at the top, locate the client document.
- Click the kiosk link adjacent to the client document on the far right.
- The Assign Client to Fill out Form window will open.
- Select the radio button adjacent to the kiosk name.
- If no kiosks are listed, verify an available kiosk exists in the Kiosk Manager module.
- Verify or select the appropriate Program.
- Skip the Form Groups and Form selection options. The clinical document number and name will display with a selected checkbox.
- The computer or tablet assigned to the selected kiosk name will display the clinical document.
NOTE: Once the clinical document has been completed by your client, you will find this document in the Clinical/Charting Module.
